Question about Tenso

I'm trying to sign up for an account on AP's Japanese site with my Tenso address using this tutorial, but I can't quite figure out which part of the address goes where on the form or which province I should select. Tenso also tells you to add your membership number to the end of your address, but I'm not sure which line to put that on either. Can anyone help clear this up for me?
